We accept referrals from organizations and agencies across the State of Wisconsin for both in-person and telehealth appointments. If you are an insurance provider, fellow healthcare provider, contracted partner organization (CCS, CLTS, IRIS), or community worker looking to refer a client or their family for services please reach out to us at:

The following information will be requested to complete the referral
- The first and last name of the individual seeking care
- Contact Information including phone and email
- Date of Birth
- Preference of Dance/Movement Therapy, Art Therapy, Music Therapy, or Drama Therapy
- Reason for referral
- Contact information of referring agency including phone and email
If the client is a minor please provide the following information from their parent/guardian:
- First and last name
- Contact Information including phone and email
- Relationship to the client
